Summer 2010 Weekly Hitting Clinic REQUIRED MEDICAL AND RELEASE FORMS These forms must be completed and brought to check-in in order for the athlete to participate. A physician must sign the medical information form stating that the athlete is ok to participate. A copy of a school physical dated within one year can be substituted for the physician's signature. A photocopy of the front and back of your insurance card is also required along with the medical form.
